Question about 1996 Plymouth Grand Voyager
At minimum it is stuck in limp mode which is to protect the transmission due to a malfuntion but still allow the vehicle to move. You need to get it to a garage and have a scan tool connected to the 16 pin connector under the dash. The stored trouble codes received will give the correct path for troubleshooting because it could be a lot of things.

Jeez---I got the exact same car and for more than 9 years it's been leaking trans fluid from seal with engine. I was told seals deteriorate faster now with increased ozone in the air. Anyway I think it's a very durable tranny. Be patient and attentive . I use LUCAS transm additive.

The solenoids may be not working correctly for shift points. Check the transmission fluid level after the car does this to verify that it is within proper range.
If the level is ok, and the transmission malfunctions, a rebuild may be required.
