Does it only happen when the car "lugs" in overdrive or 4th gear, when the rpm's are at about 1100-1300? Will it ever happen if you keep it in 3rd gear?

you have a bad torque converter. well documented on the Honda Pilot forum and Honda is ripping everyone off including me on the problem.
