That lower pullie is the water pump. Need to replace that, and put sealant around the gasget when you do.

Since the car ran well for a month after you put the new thermostat, I would say that you have a leak somewhere. Look under the car every time you park, and keep filling the coolant. It is possible that you have a more serious problem, but until you figure out what it is, this may keep you on the road. Check the fluid level every day. Tell us what happens. Also make sure your oil is not milky and there is no oil in the radiator - if this happens, you have serious problems.
