Question about 1998 Ford Escort
This is a warped brake disc rotor or a very worn set of brake pads. a) Warped and cracked brake rotors are most often caused by heat build up due to a brake caliper seizing and leaving a brake pad engaged with the rotor. The heat build up causes the rotor to get very hot and as the hot metal expands it warps. b) as the brake pads wear thin they encounter a series of warning bumps on the spinning rotor. You wil feel this through the brake pedal first. Shortly afterwards if the worn pads are not replaced the rivets holding the pad friction material begin to encounter the rotor face making a load noise. Remove each wheel in turn and inspect the thickness of the pads and the condition of the disc rotors. Any warped or damaged rotor will require that both rotors sharing the same axle will need to be replaced. Check that the calipers are capable of full travel, a seized caliper may need replacing. Renew pads as required, again a full set per each axle. Never change the the brake pads or rotors on just one side of the vehicle, changes to one side should always be matched by doing the same to the other.

Normally a "pulsing" brake pedal means that that you have a warped brake rotor or the brake caliper is lose. I would start by taking the front wheel off and inspecting the brake pads and the rotor. If the brake pads are worn down bad replace them. Pay special attention to the brake rotors also as if the are grooved very bad, they should also be replaced.
