Is it possible that the car could have came without that adapter? I see a plate that looks similar to that and there are some wires on the back side but I do not see an adapter.

Easiest and probably cheapest way is to purchase a OBD II code scanner. You can get one at autozone or online for fairly reasonable. They are good to have on hand for future repairs. Most have the option to clear codes.

Where is the OBD port located in a toyota hiace 1998 with 1KZ engine and is it a OBD II?

It is OBD II all cars world-wide were required to be OBD II by the 1997 model year. The port should be located under the dash on the drivers side of the car. It looks like a serial port on the back of a home computer.
