Start with the simple things first. When was the last time you had a complete tune up? "Missing" could indicate bad spark plugs, bad spark plug wires, or bad cylinders. Change the plugs first, then go from there.

Engine missing mainly between 45 and 55 mph

I'm going to assume you have the 3.4 V6 engine which uses direct ignition with a coil on each plug.
Could be ignition or fuel related.
A weak spark/coil will allow the engine to run on all 6 cylinders at idle but miss under load, especially when going into overdrive at 45 mph. Not enough spark to jump the gap.
Same thing with low fuel pressure or dirty injectors. Not enough fuel when the engine is working hard.
Usually when you get a hard miss you will get a trouble code for the cylinder that is not giving power.
Have you changed the fuel filter in the past 12 months ?

My car cranks but does not start. I replaced the starter but it did not make a difference. It is a 1996 4runner 2 wheel drive.

If it cranks but won't start the starter is already working which you now know.
You would have to figure out if you are missing spark to the plugs or fuel to the injectors, or low compression to proceed.

Engine starts missing when coming under load more noticeable when climbing a hill. Have replaced o2 sensors made no difference

Hello first of all

No that will not really help what controls missing is bad fuel air mixture depending on what o2 sensor you change this may or may not affect this only the o2 sensor before the cat converter will affect this as it runs mixture.

That being said here is what i would check being a mechanic for 5 years with ford and 3 with bmw first check spark in all cylinders make sure it is blue not orange do this by pulling out plugs leave them connected to wire and have someone start the care and check the color of spark.

If blue in all your good if orange in any replace the coils and or coil if you have more that one.

Then replace the camshaft position sensor and if possible the crankshaft position sensor the crank controls starting spark and the cam running spark but they work together.

If this all fails and you have had a recent tune up plugs wires coils air filter fuel filter ect

If all this fails you need to see a shop as unless you have alt of tools and time its beyond most folks reach.

I hope this helps if so please 4 thumbs up thanks

My Father in law has a 2000 lincoln town car, 81K miles, 4.6l, with coil on plug. He says that the car runs great except under load (like climbing a hill) or when it is damp outside, says the car misses....

check spark plug gaps -25th- if wide this will cause the miss ,if they are ok or still the same even after re-gapping then its a coil pack but what one ?? they are not cheap and no a code reader will tell you this information what you need is a induction sensed meter like either a nice snap on timing light if they still exist or a automotive multi test meter .

97' 4runner (4cyl) stumbles (misses) under load

Factory plugs are not always the best tool for the application. In your case, you use the vehicle for something that would be deemed as heavier usage as per your question. What you need is to contact your local dealer or Auto parts store for platinum tip or double platinum tip plugs for a greater spark, o r you could replace with the recommended pluga and regap them for a greater spark...
