Code EGR P404 is a problem with EGR (exhaust gas recirculaion), you have to replace the valve, check here for parts: partsamerica
The part costs $80/$100,. Having it done by a qualified mechanic, which is always the best choice costs around $250/$300.

Or before shelling out that kind of cash; you could try cleaning the seal on your gas cap and/or replacing your gas cap first. My '06 has tripped this code twice (after very dry or very cold weather).

The gas cap has nothing to do with the egr system on your car. the gas cap will effect the evap system not the egr system.

First check the wires going to the connector at the egr valve. After you have checked the wires remove the egr valve very carefully and clean the carbon buildup off with some B-12 chemtool, and be carefull not to damage the pintle valve. Then if you have the capability check the operation of the egr valve with the engine running with a scan tool that has bi-directional controls that will alow you to open and close the egr valve. Most likely the cause of the problem is the egr valve. I am sure that most new egr valves have the sensor built on.

SOURCE: changing fuel filter on 2004 chrysler town and country

  1. Remove or disconnect the following:
    • Negative battery cable
  2. Raise the vehicle and support the fuel tank with a transmission jack.
    • Fuel line from the front of the tank
    • Ground strap
    • Inboard side of the fuel tank straps and front T strap fastener and lower the tank about 6 inches
    • Fuel lines from the fuel pump module
    • Fuel filter
  3. Installation is the reverse of removal. Tighten the fuel filter bolts to 40 inch lbs. (4.5 Nm), the main tank straps to 40 ft. lbs. (54 Nm) and the T strap to 250 inch lbs. (28 Nm).
  4. Start the engine and check for leaks.

SOURCE: error codes 442 and 456 on 2003 Chrysler Town and Country

Could be the gas cap.  Do you fill the tank with the engine running?
Otherwise, check the hoses near the Evap canister.  I have 2003 T&C and had the small leak and large leak codes.  I looked at the evap cansiter and the hose coming from the fuel tank vapor line had a huge crack in it, to the point it had fallen off the evap caniser.  I trimmed the end of the hose and reconnected and voila, no codes.
The evap canister is on the driver's side of the van, underneath, slightly behind the drivers seat.
BTW, to clear the codes without an OBDII reader, remove the negative cable for 5-10 minutes (15 to be safe).  The computer resets and clears the codes, except for one that it sets saying loss of power to the memory.

Hi:
Both codes are EGR system codes. The egr valve can be removed and cleaned to start with, and the EGR pressure sensor is another possibility that could cause the codes.
