SOURCE: ROUGH IDLE / ENGINE SURGE
Check for vacum leaks on hose going from the intake to the brake booster. Also check the dodge.com website. Click on the for users button. Enter in last eight digits of vin and see if pcm (computer) needs a update, or if there is any other open recalls on the van.

SOURCE: Engine Surge at idle
I would recommend that you clean the intake throttle valve and idle speed control valve with carb cleaner, start the engine and hole a high idle while you spray (it will try to stall) the intake, use most of the can, then let the engine stall, disconnect the battery and wait one hour, reconnect battery and, start the engine and throttle it up good a couple of times to blow the carbon and sludge out, the idle should improve, if not you may have a hard part issue, but that is rare.

it could be a number of things. i'd go with the easiest fix first. check your air filter, then check spark plugs, then fuel filter. replace as necesary. good luck
