The engine is leaking coolant internally, via either a bad head gasket, cracked cylinder head, or cracked block.

Water in oil and white smoke indicates a blown head gasket letting coolant into combustion chambers and running down into sump. It is usually caused by allowing engine to run out of coolant, either from a leaking hose or radiator, and subsequent overheating of engine.
Only fix is to have head removed and, if head is not too corroded, machining of the head face and new gasket when replacing.

White smoke usually indicates water in the oil.

I think you can have a head gasket leak and not get coolant in your oil. You may be getting enough in there that it's causing that cylinder to not fire and give a rough idle.

WHITE SMOKE INDICATE SLIGHT HEAD GASKET.STOP SMOKING WHEN ENGINE GET HOT.HEAD EXPANDS ENOUGH TO STOP THE LEAK.BUT I SEEN CASES WHERE THE HEADS HAD TO BE PRE TORQUE AFTER ENGINE OVERHAUL.ALSO YOUR ENGINE WILL USE OIL UNTIL PISTON RINGS SEAT.SO KEEP EYE ON OIL LEVEL.

When I start my 2003 ES300 smoke comes out the exhaust

The same thing happened to my 2003 ES300 - the smoke is coming from the oil seeping into the motor's cylinders. This is the classic "Oil Gelling" problem in Toyota and Lexus vehicles. There is a factory recall on it, but it only covers to 2002 ES300 not 2003. The Lexus dealer confirmed my "oil gel" problem and went to bat for me with Lexus - they got the repair approved - Thank you Mr Local Lexus Dealer! The cost was nearly $4,000 - paid for by Lexus.

Valve stem seals can cause this problem, but normally only when first started. Oil in the rocker cover seaps through the oil seals into the cylinders and is burnt up on start up.
Try a "STOP A LEAK" additive and it may reduce the oil consumption and smoke.
I found some on e-bay and it worked on my tired old escort. It even stopped the rattling tappets.

There should be a sticker under your hood somewhere that shows how it goes on. If not, just put it on the crankshaft, and then go flat sides to flat sides, ribs to ribs, and work your way around.

Not the oil pump, the most likely cause is the intake and exhaust valve seals, this accounts for the smoke on start up

'92 Lexus Es 300 - White smoke from exhaust and smells like gas.

It's the ECU. Very common with the early '90s lexus cars. Everyone will say head gasket, but unless your oil is milky white, it's not coolant leaking into the cylinder.
Buy an ECu on ebay for less than $100, it plugs in under the dash, just like a computer printer. Easy fix.
