Book shows only 4 pins. That is all you need to run a relay. See my notes at http://www.fixya.com/cars/r6022358-relay_check and try a different parts store.

Why doesn't my low beam headlights work when all the other lights do ?

The most common cause of low beam failure on trailblazer, buick, and GMC, are the micro relay under hood in fuse box. One relay is for the low beams and the other one (beside it) is for the cooling fan. It is part number 15016745 and has four prongs on it. Just swap the cooling fan relay, with the low beam relay. This will tell you if it is bad. Then see if low beams work, If they do its a bad relay. Good-Day! Make sure to swap the relays back so that the cooling fan is working ok!

I have a 96 achieva the cooling fan only powers on at start up for a second then shuts off and does not come on again what needs to be replaced and where can I find this part under the hood?

check your coolant level. there will be a temp probe in your coolant system. if this isn't getting coolant on it (due to being too low), it may prevent your fan from coming on.

Headlights went out. Replaced bulbs and switch and still lights don't work. Fog lights and high beams work? What else can work?

If high beams work and low beams do not, then it is probably the high/low relay. It is part number 15016745 and has four prongs. To test if your relay has gone bad locate the relay in your fuse box. There will be two of these relays located next to each other. One controls the low beam headlights and the other the cooling fan. Swap the relays and check your low beam headlights. If the low beams work you have a bad relay.

2003 impala 3.8 cooling fans not coming. repaled

Check the coolant temperature sensor, it may have dirty contacts which is an easy job to fix. Signal from the coolant sensor indicates to the ECM that the engine is at running temperature and for the ECM to reduce the amount of fuel from the injectors (always a rich mix when warming up). (A little while later the thermostat should open allowing hot coolant to flow via the top hose to the radiator) The ECM rthen triggers the (5 pin) relay and the fans then come on. Check also the coolant relay fuse.

Where would I find the fan motor relay

The cooling fan relay on a 1996 to 2000 Plymouth Voyager The SSFR (Solid State Fan Relay) Cooling fan relay is located on the left front inner frame just behind the radiator. The radiator coolant fan relay is located on the inside front of the engine compartment near the left headlight assembly. You have to remove the air filter assembly, then look for a simple square relay with 4 wires running into it. It is mounted with 2 screws. It is the only relay at that location. If it has burned, you will need to replace the pigtail wiring harness.

Remove the air box beside the battery a 10 mm bolt and the relay is a four prong square box bolted on the frame with two 8mm bolts

Overheating fan will not run ! I change cooling sensor, fan relay, new radiator, thermostat, fan motor, check all fuses, tank low coolint sensor, put fresh coolant in also. (1998 achieva SL 3100 V6 SFI )...

there isa short in the wiring somewhere, which a shop can check for you,or check your thermostat. after 20 years i still have to make sure tat i dont install it upside down. which is easy to do if your in a hurry. my bet is that the wiring going to the coolant sensor has a bare spot and grounding somewhere.
