I always go for the fire, fuel & air first... but it starts... so
that's a really great indication... Could just need some fresh spark plugs... (ACDELCO R44LTS6) or a cold start valve ...

~~ 4.3L V6 Plugged EXHAUST:
I've never found an elegant way to prove it...
So we'll just jump right into...
GET some
liquid wrench Penetrating Oil and
soak the EXHAUST to CATALYTIC converter bolts
really well (couple of times).
If
YOU OPEN the exhaust and the engine runs smooth...
you have found a PROBEM that will do both thing you are experiencing...
Now you have to figure if it is the
CAT collapsing or a
MUFFLER BAFFLE blocking the
out-flow.
(It's happened to me
twice... but I'm pretty old
)

Might be easier just to take it to a muffler shop...
and have them run your
engine codes...
