I own a 2000 model myself, what ever you do do not do a transmission flush, it forces parts to be misaligned causing more damage than good. take to a mechanic and pull the trans pan replace the filter and let drain the old fashion way. inside the pan is a magnet if its coverd in large metal particles the end product may be a tranny rebuild.

Takes a while to in reverse and hesitates to go in gear. Once in gear it won't shift to 3rd or keeps slipping from one to the other

I assume this is an automatic.
This vehicle could be low on transmission fluid, or it might need the filter and fluid changed.

Gears not shifting properly, it hesitates to go into the next gear and sometime rev as high as 4 rpms but not shifting what could it be?

Automatic transmission fluid probably low. Last time you had car serviced? Proper manitenance is critical to making your car's systems last. You may have a bad leak (pink fluid) under car. Check your owner's manual troubleshooting.

My '05 altima hesitates when i shift from park to drive then jerks into gear. once driving changes gears smoothly, no jerking, no slipping, no hesitation.

Read the owners manual for checking the Transmission Fluid. Usually it involves putting the car in Neutral and checking the fluid. If it is low, that could be the problem. If it is not, it may be a problem with the gears. I would take it to a shop to try to diagnose the issue.

Just replaced trannie(auto) in 1993 escort, it has a little hesitation when shifting gears and the new oil smells burnt and is frothy on the tip. the level is low could the low fluid level be the problem?

YES! But your tranny has a BAND for first gear, and clutches for 2nd, third, and if equipted, 4th. The bands are usually the cause of the "burnt smell." The foam is cause by fluid aggitation. DO NOT JUST ADD FLUID. Drain the fluid in the tranny case, and replace the filter, and the fluid. I suspect that the salvaged tranny may have a burned 1st gear band.

I have a 1992 Ford Bronco and it began to hesitate tonight while driving. I put the gear in low and now I dont know how to get the truck back into the correct gear. Im not familiar with the floor gears

look at the driver side visor the shift pattern is printed there. it should be also on the shift lever itself . I think 2 wheel high is all the way forward then back 1 is neutral back 1 more is 4w low and all the way back is 4w high

sounds like a vacuum leak, injectors clogged, bad plugs, plug wires or maybe low tranny fluid or combo one all, have it dynoed to save yourself some money of course.
