The Ford DTC (Diagnostic Trouble Code) P2198 refers to a bias in an O2 sensor. Specifically, Bank 2 Sensor 2. It is most likely stuck reading mixture rich.

Why wouldn't it keep coming up

Not Googling your fault codes-you got the codes but do nothing

P2198 --Lack of HO2S-21 Switch -Sensor Indicates Lean

If you did the required Preventative Maintenance & replaced
the Front Oxygen Sensor BEFORE they failed at 100,000 miles,
you may not be in the situation

Every time the Emissions Monitors run, the codes come back or never left

Need a repair shop, to road test with scan tool & put a couple hours
diagnostic time into the truck & about 6 or 8 hours getting caught up
on the maintenance required

I have a 2005 Freestar Se and the P2196 and P2198 problem? Is this the fuel pump problem and how do I fix the problem?

codes p2196 and p2198 refer to the oxygen sensors
p2196 is the HO2s sensor 1 bank 1--signal stuck rich or no signal===causes HO2s --fuel pressure --injectors ---air intake restricted---wiring ---ECM
for p2198 it is HO2S 1 bank 2 --signal stuck rich===causes --same as for p2196

P0455 Evaporative Emission Control System Leak Detected (large leak) P2198 O2 Sensor Signal Stuck Rich (Bank 2 Sensor 1) P2189 System Too Lean at Idle (Bank 2) P2187 System Too Lean at Idle (Bank 1) P2196 O2 Sensor Signal Stuck Rich (Bank 1 Sensor 1) basically you need to replace either the air /fuel ratio sensor,, or,,the heater relay associated with that sensor,, these are very hard to diagnose without the hand held or scope that narrows it down ,, you can check a multitude of voltages on a number of electrical connections, as far as the P0455 ,, if you installed a new gas cap either have the code removed , or wait a couple of days and it should remove itself, as long as the cap is factory approved, Look for vacuum leaks in the evap system , cut hoses,, bad connections,

P0191 is Fuel Rail Pressure Sensor "A" Circuit Range/Performance
P2196 and P2198 are lean bank 1 and lean bank 2. You normally show lean bank codes if the air-fuel ratio is not correct. The most common cause is a hole in the hoses between the PCV valve and the intake on the motor (back of the motor generally). The fuel rail pressure sensor code could either be caused by the vacuum leak or by a failing fuel regulator. I would first check for holes in the rubber hoses especially where there are right angle rubber hose connectors. Look for soft squishy or oily spots on the hoses.

I keep getting the codes p2196 and p2198

Usually 02 sensor problem if engine running ok check for a vacuum leak.
check the PCV elbow on top of the engine block it could be damaged.
