Your engine coolant temp. sensor is going bad. What happens is that your engine computer is receiving a bad signal from the ECT sensor and it thinks it's 40 below. An example is it basically slams the choke shut if it had a distributor. When it cools off, I'll bet it runs just fine.

The fuel pump relay, which is probably by the other relays under the hood by the big fuses, can surely be tired after all these years. Don't buy an off brand, I had one of those stick ON!
Maybe the EGR valvereplace that and see what happens my truck did that and I changed EGR valve and now runs fine
