Check the following:

Coolant level (when cool)
Does fan engage when a/c is put on?
Has the engine been backflushed and new coolant installed lately?
Is the thermostat sticking shut?

Sorry, I need to ask, Is it leaking or just not putting out good heat? Not something you want to tackle, if you don't have to.

Heater in 2005 Caravan takes forever to heat and comes and goes

First thing to check is if the radiator and overflow tank are properly filled with antifreeze (with engine cool, of course) Next, is the temperature gauge moving up properly? If not, you probably need to replace the thermostat.
