SOURCE: end of coolant stack has snapped off inside hose, 1994 v6

if you want a cheap fix and you don't use the heater at all... unhook the other hose and join the two together with a fitting and two hose clamps.... if you want to fix it properly... yes you need to replace the core... the pipe is broken off completely... or just the hose ?

I recommend taking the 2003 Dodge Grand Caravan to the dealer, or a local trusted auto shop. It's a simple fix the mechanics can take care of relatively quickly.

2009 dodge grand caravan heater hose assembly replace

get a genuine hose first as it is made to fit in the right places. get a cork and plug up one end of the new hose . have a fiend fold the old hose over so that the coolant will not run out when you pull it off. put on the new hose and then remove the old hose from the other end and quickly place the new hose in place after removing the cork.. This results in minimum loss of coolant and minimum mess. Do the job when the engine is cold.. Make sure the hose clips are tight.

You say you can hear the climate control work, but the blend door for the heater may not be moving. If both heater hoses are hot, I would suspect the blend door.

Most people make new lines from straight tubing. I don't think you can buy them from the dealer after about 10 years.

If the original y-fitting is ok. Just get yourself a piece of heater hose. The auto store may have heater hose on a roll, You may need to tell them the ID and how long, they just cut a length for you and you use hose clamp. I'm not sure what you bought/an assembly? Maybe you can take it back and just get a piece of hose? Of course, I can't see anything from here?
I did find this picture.

2001 Dodge Caravan heater blows cold air when stopped

Have you checked the coolant level? What you describe is exactly what happens if the coolant level is low. When stopped at idle, hot coolant is not circulating through the heater core, so no heat. As the engine increases in rpm's when you pull away from the light, the water pump is spinning quicker, and some coolant is making it to the heater core. So check the level, and if it is low, keep an eye on it to see if it drops again. That of course indicates a leak somewhere. Get it fixed, as overheating the engine leads to costly repairs and worse case a new engine. Let me know how you make out with this.

The leak is coming from under the passenger side door and the pipes caring the coolant to the rear heater is rusted. Can this be replaced easily or closed off so that there is only heat in the front?

On my 1994 Grand Caravan:
I cut (sawed) the rusted pipes at the point of where the rust stopped (plus 1-inch. Take that cut portion of the pipe to any auto parts store and get BULK Rubber Heater Hose. Remember to add on a few inches (2) for each end to fit onto the pipe (on the Van) at each end. Use the pipe you brought to get the correct size (diameter), fitting the pipe snuggly into the rubber hose. Buy 2-adjustable band clamps for the hose, one for each end. Make sure you get the correct size diameter clamps.
Hint: When putting the rubber hose you bought onto the metal pipes on the Van : to slide the hose onto the pipe easier use grease on the outside of the metal pipe and on the inside of the rubber hose you bought (about 2-inches).
Clamp them down at least 1-inch from the end of the cut.
Replacing the metal pipes is more expensive and more labor. I did mine for about $5.00 and was done in 15-minutes, The Rubber Hoses will not rust on you.
Let me know if that helped or if you have addtional problems or questions, feel free to contact me at fixya.com
