The problem is with only the fan. The cooling system sensor either turns the fan on or turns it off.

The bearings or bushings probably loosen up after getting hot. You may can feel a roughness when you spin the fan with the engine off.

My lever that controls the speed of the ac/heat blower quit working. Sometimes the fan won't come on at all, then if I leave it on it will come on on its on. The lever doesn't work it anymore. I have a...

The blower motor has a resistor attached to it that probably has gone bad. When you use the fan select switch, power is sent through one of the resistor channels to control the speed of the blower motor.

You can test to see if the resistor is completing the ground circuit by connecting a wire from ground and touching it on one of the resistor terminals. If the blower motor comes on, then the speed control switch may be bad ( or the wiring harness is not making the connection). If the blower doesn't kick on, then try touching the ground wire on the negative side of the motor. The motor should rotate. If not the motor is bad. If it does rotate, the resistor is bad.
