Check the vacuum hoses under the hood for a leak. The elbow on the PCV hose commonly develops a hole. You may be able to hear a hissing leak. Replacement works better than any patch.
If you can't find any hose leaks, the mass air flow baro sensor may be damaged. It's in the air cleaner box. Cleaning the wires (degreaser or brake fluid) on the sensor may help.
If cleaning doesn't work it may have to be replaced.
After cleaning or replacement you should disconnect the battery ground cable and reattach it after a couple of minutes to clear the light.

Based on what the tech manuals say I would change the DPFE sensor for the EGR Valve, careful not to tear the special silicone rubber hoses that go to it from the exhaust, as a Ford dealer tech I saw these codes allot, and the DPFE sensor was almost always the issue or the hoses going to it.

These codes are dealer specific. Please call you local dealer and they will give you the exact meaning.
For now I think you may find the code needs to be verified.
Here is my advice.
Have the reading done by - Auto Zone and also Advance Auto all over again. If the codes match
Take down the vin# - call the dealer and tell them the code. Ask them what it means and - -ask if your vin# brings up and recalls or service needs as issued by the factory. Some things never go out of date.
One last this - your header here says 1995 -your note says 1998. You need to sort that out as well.
