Doesn't sound like you've considered spark. Sounds like you're losing spark somehow. You seem to know your way around a truck, so I don't have to explain every little thing. Just check all the ignition stuff, go through the distributor and coil and plugs and wires. If it's gettin fuel, its either a spark issue or air/vaccum issue. Hope this helps.

Hi, i know you said you replace fuel filter and injector are fine but have you put a fuel pressure gauge on to see what the pressure is . Fuel pump can be running but with out enough pressure it will lack power and maybe even skip from lack of fuel. See what your pressure is and compaire with car specs. this will tell you alot. if i can help you any more just re post.
Thanks dodge tec

YOU COULD BE HAVING ENGINE MISFIRE.DUE TO BAD SPARK PLUGS AND WIRES.ALSO DIRTY FUEL INJECTORS WILL CAUSE ENGINE TO ROUGH IDLE OR IDLE UP AND DOWN.PUT A BOTTLE OF FUEL INJECTOR CLEAN IN A FULL TANK OF GAS.ALSO CHANGE PLUGS AND WIRES.IF TRUCK HAS 30000 TO 60000 MILES ON IT.THATS THE SCHEDULE MAINTENCE INTERVALS IF SPARK PLUGS HAS NOT BEEN CHANGED.
