This is a failsafe mode when the computer see's something it could be minor or major. ive seen solenoid packs input sensor(no speedo from this) or to much clutch clearance(that last one is major0 GOOD LUCK

2 things come to mind. Are you 100% sure you don't have an emergency brake or any other brake sticking, even partially? 2nd thing would be Catalytic converter may be partially plugged. That is assuming a tune-up, filters etc have been done .
Wow, I really could use a little more information but here's a first help. If by "won't go into second gear" you really mean that the transmission will not shift up into any gear except the gear that its in when you put it into drive, here's what you are looking at. When Chrysler transmissions have any sort of failure that could damage the transmission even worse if you keep driving it then the computer tells the transmission to go into 2nd gear and stay there. This allows you to get off the highway and get to a shop or call a wrecker. If you put your van in OD and take off and it won't shift then you have an Issue. Now, sometimes, and I really mean some times, you can disconnect the battery and if it was just a fluke then you will be good to go. If it happens again in the near future then you should definitely have it checked at a transmission shop. Just unhook the negative battery terminal for a few minutes, hook it back up and try it. Most automatic transmissions will have a cooler that is in the radiator. Some will also have an external cooler (really common on vehicles with a towing package). It is really common for the cooler hoses to leak on the caravans. Make sure you are using ATF+4 for trans fluid. No other transmission fluid should be used in a chrysler!
The transmissions available in your caravan are problematic for having a bad solenoid pack. Once the transmission is set in the LIMP IN mode, then you need to take it to a dealer to get the code reset from the transmission and to get it diagnosed properly. I'm sure there will be a code such as, "trans solenoid stuck in LR" which means " low range" if the transmission went into limp in mode, then that simply means it defaulted to second gear only. Which is telling you to have it looked at asap. I'm sure if you repalce the transmission solenoid pack and gasket and perform the re-learn procedure, then your problem will go away. good luck!
