What could cause the car to stutter when under load going up hill.

It sounds to me like a passing gear issue. Or even and Over Drive issue. Try this to find out. When you put the car in gear, instead of O/D, try regular drive. If you have a push button O/D then deactivate it. If you only have Drive, then try shifting into 3. Run the car up to the same speeds you were having problems with. If you don't have the issues with the higher RPM's then it is an Overdrive/Highway gear issue. If it still stutters, then it could be a fuel delivery issue. Good luck.

What could be the cause of a rattling noise but engine never shuts off. No Mil. Alerts or any stuttering. just loss of power and loud knocking noise the more you rev it?

I have had personal experience of a head gasket blow the dodgy mechanic who re-built my motor at the time did not clean the head bolts and so they were not tensioned properly (I digress). Anyhow it blew between two cylinders - man what a racket! I thought my engine was kaput. In short my answer is head gasket - could also be a burnt out valve but most likely something in your top end of the motor.

Engine stutters or stalls after a complete stop upon engaging to go forward

You'll need to have someone scan the computer for the codes.
Most auto parts stores will scan for free in hopes of selling you some parts. Once you get the codes you can research them online or in this question.

Car starts ok will idle but when you go to drive over 1500 revs car stutters and blows out clean smoke (fule not oil)

Does it overheat at all?

White smoke? or Dark grey smoke?

Could be Oxygen sensor (grey) or head gasket (white).

Mazda MPV 2004 - Stutters, check engine light came on

chances are fairly good it's the coils - I've posted about this on Edmunds. the OEM design is truely idiotic & has a vent that allows moisture down into the plug well - that shorts the spark & causes misfires. On our 2004 MPV I got away w/cleaning & drying them extremely carefully then sealing the vent w/silicone as a temp measure, then ordered perfect replacement after-market coils off ebay that sealed the wells & stopped the problem -and- were a tiny fraction of the OEM price.  good luck, BK
