Sounds like a thermostat and/ or the system is low on coolant.
Its probably not going to reach 200 degrees when the outside air temp is below freezing, but you should get plenty of heat if the system is working correctly.
coolant fan should kick on when engine coolant reaches 190 to 200 degrees.the coolant temperature sensor resistance will drop.closing contacts allow the pcm to signal or turn on coolant fan.if coolant temoerature sensor is open or bad the coolant fan will not turn on.need to replace it.
Can't give you a definite "fix" only because I'd need to do testing. But, two items you need to have checked are: 1. have a hydrocarbon test done fro the cooling system to verify that you do or don't have a head gasket problem. 2. have someone check the radiator with a "heat gun" to see if there are any cold spots. Home flush kits and sometimes even shop flush equipment will force cleaner around, not through heavily plugged areas and not clear the obstruction. Heat gun is best way of finding cold areas that are not flowing. also...you mentioned fan relays...does the fan come on?
it could be your thermostate . i do dont think its your intake.as far as putting the cap on to tight.. they are sopuse to be tight so you did not hert it . have a nice day
