severe engine misfire , Plugs , plug wires , possible plugged catalytic converter . Shifting problem possibly bad shift solenoids inside transmission , With all your problems isn't the check engine light on ? Your best bet is to take that vehicle to a qualified repair shop .

Could be fuel filter. When you need more gas for power going up hill your engine can't get enough.

Car jerks when accelerating

Jerks as in fuel starvation or jerks as in transmission slippage? Do you notice it more going up hills than you do on flat ground or going downhill?

Most likely is a fuel flow problem so change fuel filter and do a pressure test on the fuel pump. But also make sure that you don't have water in your fuel - if you think that might be the case - then pour a can of heat into it.

It jerks real heard nd cuts off

Going to need more of a driveability description, than it jerks.

When,what speed,what rpm, up hill, under a load,what gear you in,etc.

Unplug EGR Valve, plug vacuum hoses, and road test, see if issues goes away,if it does after a couple trips, replace valve yourself.
