The problem is most likely to be with your vacuum line they put cheap rubber hose for this purpose. You should find a diagram at the front of the vehicle trace the hoses by this diagram. You most likely will find a bad piece of hose.

The error code P0135 ,this error code is related to O2 Sensor Heater Circuit Malfunction (Bank 1 Sensor 1).It would be a upsteam o2 sensor. The upstream one which is mounted on the exhaust manifold. The sensors is located in the back of the engine on the exhaust pipe. You must have a O2 sensor socket to replace it because of where it is located. You can borrow one of these from Autozone. The sensorlooks like a sparkplug with 2 wires coming out the top of it. The error code P0403 , this error is related to Exhaust Gas Recirculation Circuit Malfunction. For the details about this error i suggest you to click the link shown below. The link :--- http://www.obd-codes.com/p0403 ------------- The error code P0442 ,this error is related to leak in the Vaporative Emissions or EVAP system. Common problem. Few places to check - rubber hose under air box (requires removing the airbox) and hoses connecting to evap system under the van on the drivers side. -------------- The error code P0445 .This error indicates Evaporative Emission Control System Purge Control Valve Circuit Shorted.Try replacing the gas cap.If its loose then tight the gas cap and check. The charcoal canister stores gas fumes from the gas tank. Every so often, the computer commands a "purge valve" to open up and draw fresh air through the canister to remove the gas fumes. The engine sucks those fumes into the intake manifold and burns them. The purge valve is an electric solenoid valve. Actually, there are two of these valves--one is the purge valve that opens the vacuum line between the canister and the intake, the other is a vent valve that opens the canister to take in fresh air. The computer monitors the voltage on the lines to the valves and watches for a voltage drop when it commands the valves to open. If it doesn't see that drop, it stores a code. ---------------- This are the details of the error codes mentioned by you.The mentioned parts has to be checked and replaced to clear up the problem. -------Thanks.helptech.
