You don't need to take the bumper off, it should just slide up of the engine bay. Just make sure all the lines attached to it (including the transmission oil ones)  and the electric wiring for the fan are properly disconnected. Full procedure:
Raise and safely support the vehicle.
Remove the radiator outlet hose from the radiator hose retaining clip and remove the retaining clip from the shroud.
Remove the tranny cooler lines from the retaining clips on the cooling fan module shroud.
Lower the vehicle.
Remove the entire air cleaner assembly and the air intake resonator.
Disconnect the fan motor electrical connector(s).
Disconnect the mounts to the upper radiator from the radiator crossmember.
Remove the upper radiator crossmember.
Remove the cooling fan module mounting screws.
Disconnect and plug the transaxle oil cooler line from the radiator fitting on the lower left side.
Raise and safely support the vehicle.
Remove the filter/drier, cooling fan module and radiator mounting bolts located on the lower right of the cooling fan module.
Lower the vehicle. Remove the upper cooling fan module-to-radiator retaining clips.
Remove the radiator from the vehicle.

On second thought instead of searching for the correct bolts you should buy the repair manual and follow it

2001 chrysler town country ex 3.8 v-6 how to put on a radiator fan relay? what does the relay look like?

the radiator fan relay does not look like a relay at all. on this car is a solid state relay meaning there are no moving parts. it is a black plastic box with small metal rods sticking out of it in a grid (for cooling). it is located on the metal bumper (not the one you see from front of vehicle, the one behind it) you can get to it by removing the grill or just taking out the top screws and pulling it back.

Low speed fan relay for the 2005 chrysler town and country

If you are talking about the cooling fan relay for the radiator there is no low speed and high speed relay. There is only one relay and it is behind the grill revitted to the bumper rail. It is doesnt look like you typical relay.
