You will need: New bearings, 1 pound of heavy grease, Jackstand, jack, Lug Wrench, appropriate open-end, box and/or sockets and rachet.
First, Take the bearings out of the box and paper. Force grease into every opening until it come out from everywhere, fill them completely. Second.. After jacking your vehicle and supporting the PT with jackstand(s), remove the tire, Now you have to remove the brake caliper assembly. There are two long bolts holding the caliper assby to the bracket. Tie up, out of the way, do not remove brake lines!Next remove the cotter pin, and castle nut holding the brake disc to the axle. You should be able to remove the disc. Remove the old bearings from axle and disc respectively.Take the inner bearing out of the grease and place it on the axle, holding the disc stright to replace it over the bearing. You must then, while holding the disc, place the outer bearing on the axle and follow it with the retaining washer and castle nut. Replace the cotter pin and cap. Replace the caliper assembly.
