I have a 2004 ford explorer xlt. the indicator says engine fail safe mode. what could be the problem?

Failure Mode Effects Management (FMEM) is an alternate system strategy in the PCM designed to maintain engine operation if one or more sensor inputs fail.
When a sensor input is perceived to be out-of-limits by the PCM, an alternative strategy is initiated. The PCM substitutes a fixed value and continues to monitor the incorrect sensor input. If the suspect sensor operates within limits, the PCM returns to the normal engine operational strategy.
All FMEM sensors display a sequence error message on the scan tool. The message may or may not be followed by Key On Engine Off or Continuous Memory DTCs when attempting Key On Engine Running Self-Test Mode.
With the fail safe mode was the "Service" wrench light lit or check engine light ?

Failure safe mode with wrench, limped back to work today. what could cause this?

run fault codes to find which sensor /servo put the engine in limp home mode. Check engine oil level and coolant level at the radiator.

Is the battery dead?

At least try & get any codes present
before disconnecting it, as you have a wrench light on

That you should know about-system malfunctions & check
lamps have been around for 32 years

Failsafe is either a really bad ignition coil & class A Misfire
that could ruin the exhaust converter or an overheating problem

If you replaced the battery already, read your owners manual
to see how to get the security system clear, if that is even
applicable to your truck. My Lincoln you can do whatever you
want & have non of those issues with batteries & PATS security

Engine safe mode with wrench symbol. coolant was low but i tap it off light goes off but came back on after two days. coolent is normal this time when it came on

That should be the drive by wire...remove the duct work to get to the throttle plate carefully open it up and clean inside the throat where the throttle plate is ..use a little brake cleaner on a rag and wipe it out..dont use and kind of wire brush ..sometimes they stick a little and the computer throws it into safe mode

safe mode means the computer is protecting the engine by shutting down some operations - like half the cylinders.

go to autozone & have codes read (DTC) free.

I do not know about a Jag. But the "fail safe engine mode " on my GMC means the Engine Controll Computer has quit working.

Looks like you have acommunication problem with the computer, it is going in to safe mode. Check all computer connections before you go to jaguar.
I have attached a picture of the error code.
