sounds like the diaphragm in the horn is worn/dried out. check to see if you have battery voltage to the horn with either a test light or voltmeter and the horn has a good ground as if not then the horn won`t operate correctly. if these are good replace horn.
horns are only a magnetic coil actuated by a set of points that make and break a circuit at a rapid speed there by vibrating a tin disc that produces the noise we hear. If it continually blows the fuse then the coil is shorting out (probably rusted) and the answer is to buy a new horn.
My horn is low when I press on it it makes a little toot noise would I need to replace the horn I'm thinking it's a fuse or relay it makes a low pitch sound where you can barely hear it thanks in advance
first check the fuses if they are good check for power at the hrn while someone pushes on it the wheel, if you have power replace the horn if no power you have to follow the wiring until you find a break
