My oxygen sensor was replaced and malfunctioned

Oxygen sensor was replaced with a Bosch one..then it malfunctioned according to the computer...then it was replaced...malfunctioned again. Mechanics took wiring apart to see if that was the reason...they replaced it again but this time with a DELCO brand and now I'm going to see if that worked. If not they said then my computer is bad on the truck., 2004 chevrolet colorado pickup

On certain vehicles there are two O2 sensors(upper & lower) i hope yours is not one....replacing one doesnt fix it, both must be replaced,
also make sure the O2 wires aren't burnt any place along the lines from the exhaust...Delco unit hopefully should be the answere?

P0421 code


"FWIW, according to the Mazda factory service manual, the P0421 code indicates "Warm up catalyst system efficiency below threshold".

The DETECTION CONDITION for this code indicates "Number of heated oxygen sensor (rear) inversion becomes closer to that of heated oxygen sensor (front)".

POSSIBLE CAUSES are:

Warm up three way catalytic converter deterioration
Leakage exhaust system
Heated oxygen sensor (front) malfunction
Heated oxygen sensor (rear) malfunction
Mass air flow sensor malfunction
Throttle position sensor malfunction
Vehicle speed sensor malfunction"
I copied the above from a discussion group on the PO421 code. The code can be caused by any of the above listed problems. The vehicle should be further inspected and diagnosed, so you do not have components replaced that do not need to be replaced.

Oxygen Sensor problems... P0135 (Bank 1, Sensor 1) and P0141 (Bank 1, Sensor 2)

P0135 - Oxygen O2 Sensor Heater Circuit Malfunction (Bank 1, Sensor 1)
This code refers to the front oxygen sensor on Bank 1. The heated circuit in the oxygen sensor decreases time needed to enter closed loop. As the O2 heater reaches operating temperature, the oxygen sensor responds by switching according to oxygen content of the exhaust surrounding it.

The ECM tracks how long it takes for the oxygen sensor to begin switching. It the ECM determines (based on coolant temp) that too much time elapsed before the oxygen sensor began operating properly, it will set P0135.

Symptoms: You will likely notice poor fuel economy the illumination of the MIL.

Causes: A code P0135 may mean that one or more of the following has happened:
O2 Heater element resistance is high
Internal short or open in the heater element
O2 heater circuit wiring high resistance
open or short to ground in the wiring harness

Possible Solutions:
Repair short or open or high resistance in wiring harness or harness connectors
Replace oxygen sensor (cannot repair open or short that occurs internally to sensor)

Note: Typically a failed catalytic converter does not cause this code. You're more likely to see a P0420 code for a failed converter.

Possible Solutions:
Replace oxygen sensor (cannot repair open or short that occurs internally to sensor)
Repair short or open or high resistance in wiring harness or harness connectors

Replace catalytic converter


The oxygen sensors are easy to replace which takes a 22mm wrench or a special removal tool. The converters are the problem. These to my knowledge have to be welded on. A local shop usually sells aftermarket which are just as good because these have to meet the same standards. For your accord you should need one converter. If the Accord is a 4 cylin it will take 2 oxygen sensors and 6 cylinder takes 3. Usually there are one sensor at the manifold and one at the converter. I would buy aftermarket sensors. I work at Bosch and operate the line that makes these. These look different than your OEM but are actually the same or better. Aftermarket is about $40, OEM is about $70-110. You can buy the converters online cheap, but buying from a shop you can at least show up there if you have problems. You can replace the sensors after the converters are replaced which takes about 10 min to replace. Good Luck

Here are a few causes, that may have contributed to this failed sensors actions:

A.Open or short to ground in the wiring harness(CHeck the sensor harness for this type of fualt)
B.O2 heater circuit wiring high resistance(Replace this sensor if this exsist)
C.O2 heater element resistance is high(Replace the sensor if this exsist)
D.Internal short or open in the heater element(Replace the sensor if this exsist)

NOTE_Possible Solutions

A. Replace oxygen sensor (cannot repair open or short that occurs internally to sensor)
B. Repair short or open or high resistance in wiring harness or harness connectors

I have a 2003 3.0 engine Honda Accord that is giving me trouble codes P2197 P0154 P2254, what do these mean? Thank you.


Here it goes.
P2197 - Front Air Fuel Sensor (O2 sensor) Bank 2 - Sensor 1 Signal stuck lean.

P0154 - Front Air Fuel Sensor (O2 Sensor) Bank 2 - Sensor 1 Heater System Malfunction.

P2254 - Front AIr Fuel Sensor (O2 Sensor) VS Line high voltage.

IF Trouble codes P0134 or P0154 are present diagnose them before code P2254.

The first thing I would do would be check the heater circuit. You will have ground, and 12V+ to the 02 sensor heater line on a cold start. This heats up the oxygen quicker and allows it to operate in the correct amount of time. 9/10 times if there is a heater circuit code it is the oxygen sensor. But 1/10 times it is a blown fuse. So checking the voltage at the sensor is a quick test for P0154.

I would backprobe the signal line when the vehicle is at a full operating temp with a graphing meter (if avail) or use a DVOM. You should see it switch between .1 volts to 1V. It should not continuously move up and down. This indicates the computer is in good fuel control.

Diaginosis the heater circuit first is best because code P2197 can be set from anything that can cause that motor to run lean. (Plugged fuel filter, low fuel pressure/volume, vacume leak).
