My toyota hilx 4 runner 4y engine starts but it idles rough and when i apply throttle it dies and wont rev or run right points have spark backfires

The only thing left is the cam lobe it self. Run all new plugs for just one week. Pull the plugs - laying them out each plug for each cylinder - -mark them. Change them to NEW again - in one week - pull them and compare them with the new plugs. Some times you just get back plugs too so if you want to short cut it - change the plugs say - that cylinder with number 3.
Also - how old is the timing chain? This happens a lot with stretched chains - That is an old engine so I would seriously consider the timing chain being stretched. Inspect it as a precaution. I would replace it if you do not remember when it was changed to the month and year.

Don't worry about actually pointing toward # 1 cylinder, just the tab under cap that goes to # 1 spark plug wire. # 1 cylinder needs to be at TDC on compression stroke, and rotor pointing at # 1 wire tab under cap.

It has a misfire ,we changed the plugs and the coil pack and wires still has a misfire what else could it be

Gas or compression. Check the injector circuit on 1 and 3. Are they pulsing, to put gas in the cylinders? Do a compression test to rule out loss of compression as cause for the misfire.
Kind of odd it would be 1 and 3, but that is one right after the other in the firing order of, you have a single coil? So you have a distributor-examine closely the distributor cap and rotor if those were not changed too, with the plugs and wires. Look for damage, little black tracks that indicate shorting of the spark, maybe damage at the electrode tips on the inside of cap on 1 and 3 plug wire towers. Most people change the cap and rotor when doing the plugs and plug wires.
Just a possibility also, that the injector ground wires for 1 and 3 might share the same ground. That is to say, they might splice together before going to the computer where the computer internally grounds injector circuits to cause pulsing. Well, those are my thoughts, good luck. have the intake gasket checked. spray carb/choke cleaner around intake gasket and if rpms change then you have a bad gasket which causes rough idle bad fuel econ

The igniter interprets signals of time and position from the distributor, fits the efi ecu with information and then the ecu gets back to igniter to tell it when to fire specific cylinder, we used to have problems with the distributors and also igniters.

Your injectors may be shot ... That might explain the code. As horrible as this is to ask, you seem pretty handy with your tools, do you by chance have a carburetor, perhaps a quadrajet or whatnaught, to hook up just for a second to see if your ECU/fuel injection system is FUBAR'd?

What is the proper plug wire order on my distributer cap?

18436572 to the left of the square plug on the cap is # 1 plug ( where the power plug and distributor plugs are ) then clockwise from that is 8436572 assuming you have the 5.0 or the 5.7 engine the rotor has to point at the driver side front plug on engine thats number 1 with the engine at top dead center plug out engine on compression stroke timing mark on tdc or 0 degrees

If # 1 cylinder is on compression stroke and at TDC, the ignition rotor should be pointing at the tab inside cap for # 1 spark plug wire.

How to start the firing order on the distri butator cap

Turn the #1 cylinder to top dead center, on the compression stroke. Remove the cap and see where the rotor button is pointed. This is the #1 cylinder plug wire position. Start from here, and clockwise run 1-8-4-3-6-5-7-2.
