If a change point is specified it will be found in the vehicle service literature supplied with the vehicle when new - or give the dealer a call.
Timing chains are more durable and longer lasting than timing belts - as long as the maintenance intervals are observed and good quality engine oil is used. Mostly replacement is required when the chain becomes noisy though some installations are so efficiently guided and tensioned a noisy chain, even when badly worn, is unlikely and so it is wise to remove the top cover periodically after a high mileage to check the chain for wear over the cam sprockets.
ONly sign I would use is if it gives a mileage in the maintenance portion of your owners manual. Most times it is timing belts that need replaced at 80K miles or whatever but that isn't necessarily the case with timing chains.
No other real sign on when to change them other than when the fail and when that happens you jump from replacing the belt/chain to replacing the engine.
Once you press the accel pedal what happens. can you please let me know so I can help you out
No accel at all, is the throttle plate moving if you see inside inlet manifold, does she leak any vacuum, have you check compression ,
let me know

That V-6 has a timing chain.There should be no reason to replace it at that mileage,assuming normal maintenance and no extenuating circumstances.These don't have problems like the old V-8s that would break the upper timing chain guides.I like these engines.We do replace a lot of the ignition coils and chase around electrical problems on a regular basis.
A Nissan ultima does not have a timing belt,it has a timing chain,look up in your manual for your maintenance schedules when and at what mileage you are needed to change the chain,usually the chain is changed when it is loose through stretching in wear over mileage,a very loose chain has a rattling noise in the engine compartment by the cam gears,good luck ,hope that I was helpful.
Follow your owners manaul, it has all the services needed according to mileage, of course if you havent been following the book, it doesnt do much good, and you are overdue for Coolant, Trans fluid/ filter, belts, filters, ect. Your car should have a timing chain.
The fault mentioned can be related to many issues but it will be good to have the vehicle checked for all the wrong codes, done the maintenance for the mileage clocked. Use good grade fuel, replace air filters, fuel filters, get the timing/ calibration done.
There is no recommended mileage, when the engine fails and needs overhaul would be the only time the chain would be replaced, and that would depend on the condition of the chain and associated parts. These 4.6L V8 run hundreds of thousands of miles in most cases with little trouble but regular scheduledmaintenance.
