it is egr flow range/performance in the rear top of engine more towards the left side there is an egr valve, also on the hood or radiator support a sticker with all the emm. components in a map.look for egr and it will show you the position of it.
the vacuum hose cracked or fell off repair as needed
if not a vac. problem and valve checks good then the passages in the manifold are plugged and need to be cleaned
