check switch for proper operation //check motor control for power ,connections and wires are not damaged or disconnected===check seat track for debris or something stuck
You know, I had that exact problem when a nickle fell down between the seats and blocked the track the seat runs on. Another time, a penny did the same thing on another car.
A test would be to unplug the harness under the seat and see if the seat was getting "juice" when the button was pushed to make it go "forward".

the differential or( third member) in the transmission has thrown out the diff pin. the pin that hold in the spider gears. the transmission is junk and probably has a big crack in it now from when it initially shot out of the differential. when you spin the front tires alot on a front wheel drive cars this pin will come loose and shoot out the side of the transmission.
leaking fluid will not cause a brake to lock up unless the wheel cylinder is leaking and caused the brake shoe to swell. If so, you need a new wheel cylinder.
