When the car is cool i would remove the coolant (radiator) tank cover and start the car then check to see if there is flow in the tank and see if the coolant level is really low if so fill it up to the proper level but ifthis is ok look to see if there is any flow in the radiator you should be able to see the water flowing around in the radiator or tank.
If you do not see any flow i would check the water pump outlet hose to see if there is any pressure if not then i would try to replace the water pump but if there is good pressure I would suspect the thermnostat it could be stuck shut thus not allowing the water to flow through the engine. in this case replace it.
And finaly if the radiator it'self is clogged it wont let the water circulate thru the system thus overheating the engine and you might even have a faulty thermostate temperature sensor but since you are saying that the fan is kicking on like it should i would not suspect this ok

ok if youare seeing bubblees then you definatly have a problem either in the pump area or you could even have a problem in the cylinder head gasket please check the oil to see if it is a whitish color if so bad bad you might have a gasket that has blown or worse but if all checks out good than you have a leak probably in the gasket for the water pump or one of your hoses are drawing in air somewhere ok so please check them too
Also when the vehical is running see if you can see any visable water leaking in the vicinity of the water pump area if it is in the middle of he pump area than the pump is going out on you and needs to be replaced ok

I forgot to add one thing if your thermostat gasket is leaking this will also make bubbles in the tank so you might want to check this out too ok

I have a 1996 Previa and the same problem.I replaced the thermostat and the water pump,it was ok for about three weeks and started overheating again. This time we found out that there is a pin that goes in the water pump and it fell out. Unfortunetly the van hasn't been worked on again so I don't know why the pin fell out or how to fix the problem.
