Sounds like the belt tightener unit is wore out and needs replacing.
As the bearings wear out the pulley will move around causing the belt to come off. When you replace the tightener you should replace the belt as well.

Nearly all the steps are the same - including jacking the engine & removing the mount bolts. You can skip the part about changing the tensioner pulley if you are not replacing it.

The belt tensioner is spring loaded and needs to be rotated counter clockwise to release the tension.
Pay attention to the belt routing before you remove it. Good luck

Loosen engine mount then jack engine up off mount and remove belt and replace! Lower engine back down on mount and re-torque nuts.

Serpintine belt broke . seems to have seized part on the bottom of the belt(the part that the belt tensioner sits on). need to replace belt.

your a/c compressor is probably seized up. have to replace or route belt around (but your a/c will not work.)
