Apparently, the most common cause is a loose gas tank cap. It has to be tightened until it clicks. Other, more serious faults can cause this indication, but the loose cap issue is widespread. Hopefully, that's all it is in your case!

The outer cone is where the problem is, right where the fiber/foam of the cone material attaches to the outside metal frame. if you look closely, you will find the speaker is torn along this line, at least at some point(s). I used ge silicone 2 clear, make a bead all the way around, took about ten minutes, could not be better. Great fix. Also check lexus owners club for more detail.

About the only way to try to remove all the coolant from the trans is to do several power flushes. But, whether flushing the system or repeatedly draining and refilling the system, odds are very good that damage is done to the transmission. This is because the coolant washes off the transmission fluid which is vital to keep the bands and gears properly lubricated, much the same as when coolant washes oil off engine bearings from a blown head gasket and causing internal engine damage.
At this point, ask them to use a transmission flush machine to flush out the system and see how transmission is, but you very well may be looking at either a trans overhaul or replacement.
I wish I didn't have to be the bearer of bad news, but the above has been my experience in dealing with transmissions that get coolant in them.
