A lot of vehicles have nothing but the wheel nuts holding them tight because the actually caliper will hold them tight and aligned as well. What I would be concerned with is the fact that the diameter of the lugs seems larger. I would strongly suggest trying to give the rotors back to the shop and getting your money back. I would then buy a good set of your own from AutoZone or someplace and that way you can make sure you are getting the correct rotors. Brakes are too important to mess around with. If it is a reputible shop they will let you give the rotors back because obviously there is a problem with the one rotor anyway. If they won't let you give the rotors back, I would demand that they replace the ones that they have put on there as the squeaking is directly related to the one rotors placement. Good luck! Lift the front of your Ford F-250 off of the ground using a jack and jack stands. Remove the wheels from the Ford with a lug nut wrench to prepare the truck for servicing. 2 Open the hood on the engine compartment and take the lid off of the master cylinder that bolts to the brake assister on the driver's side firewall.3 Remove the hex-head bolts on the back of the brake caliper with a socket. With the bolts removed, pull the caliper off of the Ford's brake rotor.4 Pull the old brake pads out of the caliper and discard them. Squeeze the brake piston back into the backside brake caliper with pliers, and then insert the new pads into the brake caliper.5 Pull the rotor cap cover off the middle of the rotor with a screwdriver and use a socket to remove the nut located behind the cover.6 Pull the rotor off of the wheel spindle.7 Remove the old wheel bearings from the brake rotor and insert them into the new rotor. If the old wheel bearings are worn, replace them with new bearings.8 Slide the new rotor onto the wheel spindle and secure it using a bolt and socket. After you tighten the bolt, press the cover back onto the rotor.9 Place the caliper on the rotor and secure it with a socket and hex-head bolts. 10 Duplicate the process on the passenger side of the F-250. 11 Close the master cylinder after putting the wheels back on the Ford and lowering it to the ground. Press on the brake pedal to seat the pads against the rotor Replace the F-250's Rear Brakes 1 lift the rear of the Ford F-250 off the ground with a jack and jack stands. Remove the wheels from the Ford with a lug nut wrench.2 Place a strip of paint on the back of the brake drum and rear brake drum cover so you can position the brake drum orrectly when you put it back on. If you plan to install new brake drums on your Ford F-250, skip this step. 3 Place the brake drum puller on the brake drum and pull the drum from the wheel hub. The method for attaching the brake drum puller depends on the brake drum puller that you use. 4 Remove the springs that attach the brake shoes to the wheel hub by pulling them off with pliers.5 Lift the old brake shoes off of the wheel hub and place the new brake shoes on the wheel hub. Secure the new shoes with the springs.6 Pound the brake drum onto the axle hub with a rubber mallet.7 Duplicate the process on the passenger side of the Ford.8 Put the wheels back on the F-250, lower it to the ground and, then press on the brake pedal to seat the shoes against the brake drum.

3. spray center hole of rotor with liquid wrench and allow to sit for 5 or 10 minutes.
4. if you are replacing rotor, take a hammer and beat on the backside of it and it will pop off.
5. If you are machining the rotor and re-using it, use a small 2x4 to beat on so you do not damage the rotor.
CAUTION: Do not use heat to loosen a seized lug nut . Heat can damage the wheel and the wheel bearings. Loosen the lug nuts.
Remove the center cap.
With the weight of the vehicle on the wheels, loosen the lug nuts.
WARNING: THE ELECTRICAL POWER TO THE AIR SUSPENSION SYSTEM MUST BE SHUT OFF PRIOR TO HOISTING, JACKING OR TOWING AN AIR SUSPENSION VEHICLE. THIS CAN BE ACCOMPLISHED BY TURNING OFF THE AIR SUSPENSION SWITCH LOCATED IN THE RH KICK PANEL AREA. FAILURE TO DO SO MAY RESULT IN UNEXPECTED INFLATION OR DEFLATION OF THE AIR SPRINGS WHICH MAY RESULT IN SHIFTING OF THE VEHICLE DURING THESE OPERATIONS. Raise and support the vehicle.

Remove the lug nuts.

CAUTION: Replace the lining if worn to or past specifications. Replace the brake shoes and linings in complete axle sets. Inspect the brake shoes and linings for wear or contamination.
WARNING: BRAKE FLUID CONTAINS POLYGLYCOL ETHERS AND POLYGLYCOLS. AVOID CONTACT WITH EYES. WASH HANDS THOROUGHLY AFTER HANDLING. IF BRAKE FLUID CONTACTS EYES, FLUSH EYES WITH RUNNING WATER FOR 15 MINUTES. GET MEDICAL ATTENTION IF IRRITATION PERSISTS. IF TAKEN INTERNALLY, DRINK WATER AND INDUCE VOMITING. GET MEDICAL ATTENTION IMMEDIATELY. Disconnect the front brake hose (2078).
Remove the front brake hose bolt.
Remove the copper washers. Plug the front brake hose.
Disconnect the front brake hose
Remove the disc brake caliper (2B120).
Remove the two caliper bolts.
Lift the front disc brake caliper (2B121) off the front disc brake caliper anchor plate.
On 4x2 vehicles, measure the front disc brake hub and rotor (1102) thickness.
Replace the front disc brake hub and rotor if not within specifications 27.7mm Min.
On 4x4 vehicles, measure the front disc brake rotor thickness.
Replace thhe front disc brake rotor if not within specifications. 27.7mm Min.
Your front brake rotors are warped. This can be caused by over-tightning the lug nuts or by high speed stops that overheat the brake rotors. You will have to remove the rotors and have then turned at a machine shop if possible. Install new pads when you do this as you should not use the old pads on the newly turned or replaced rotors. When installing the wheels make sure to torque the lug nuts to exactly 100 lb/ft and tighten them in a star pattern.

Dis brakes have residual pressure thar keeps the pads in constant contact with the roter.It is very light contact, so not to worry. If left is not in contact with rotor then chk it for loose wheel bearings.

Removal
Raise vehicle on hoist.
Remove wheel and tire assembly. Be careful to avoid damage or interference
with caliper bleeder screw fitting and front disc brake rotor shield (2K004).
CAUTION: Handle
front disc brake rotor (1125) and disc brake caliper (2B120) assembly in such a
way as to prevent deformation of front disc brake rotor , and nicking,
scratching or contamination of brake linings/rotor surfaces. NOTE: To prevent damage, do not allow disc brake caliper to hang by the
front brake hose (2078) .
Remove caliper anchor plate bolts and discard. Position disc brake caliper
out of the way. Prevent separation of caliper housing from anchor bracket
because contamination of guide pin journals can occur.
NOTE: If excessive force must be used during front disc brake rotor
removal, the front disc brake rotor should be checked for runout prior to
installation.
If additional force is required to remove front disc brake rotor , apply Rust
Penetrant and Inhibitor F2AZ-19A501-A or equivalent meeting Ford specification
ESR-M99C56-A on front and rear rotor/hub mating surfaces. First, strike front
disc brake rotor between lug bolts (1107) with a plastic hammer. If this does
not work, attach 2-3 Jaw Puller 205-D027 (D80L-1013-A) or equivalent and remove
front disc brake rotor . Remove front disc brake rotor from wheel hub (1104)
assembly by pulling it off the lug bolts.
Installation
If original front disc brake rotor is being installed, make sure rotor
braking and mounting surfaces are clean.
CAUTION: Failure
to clean rust and foreign material from rotor and hub mounting faces when
installing new or old front disc brake rotors will result in high rotor runout,
which will speed up the development of brake roughness shudder
vibration.
Apply a small amount of Silicone Brake Caliper Grease and Dielectric Compound
D7AZ-19A331-A or equivalent meeting Ford specification ESE-M1C171-A to pilot
diameter of front disc brake rotor.
Install front disc brake rotor on wheel hub assembly.
Install disc brake caliper and new caliper anchor bracket bolts on front disc
brake rotor . Tighten caliper anchor bracket bolts to 88-118 Nm (65-87 lb-ft).
CAUTION: Failure
to tighten lug nuts (1012) in a star pattern may result in high rotor runout,
which will speed up the development of brake roughness, shudder and
vibration.
Install wheel and tire assembly on vehicle. Tighten lug nuts to 115-142 Nm
(85-104 lb-ft) using Rotunda Accutorq® Lug Nut Socket 164-R0303 or equivalent on
a 1/2 inch air impact wrench, or use a torque wrench.
Lower vehicle.
Pump brake pedal prior to moving vehicle to position brake linings.
The shaking would be from your front rotors. First, loosen the lugs and retighten them with a torque wrench (not an impact gun) to the proper setting as described by Toyota. If the lugs are tightened unevenly or too tightly, it will artifically warp your rotors. If this does not fix the issue, your rotors are most likely warped from overheating, or being sprayed with water while hot, or from age. Get new rotors (and pads - always use new pads with new rotors), install them, and torque your wheels properly, and you should be in good shape.
