Question about 1994 Pontiac Trans Sport
It's the blower motor located on the fire wall. Easy to replace. Just did mine. Make sure you have a wheel though when replacing. There is a blower with a wheel and one without. The one without will make your vehicle sound like an airplane.

From your description, it sounds like your fan is the problem? I'm not intimately familiar with the pontiac vans but you pretty much need to track down the heater fan motor and the wiring between it and the switch. Check these wires for a fault and/or replace the wiring or the switch or the motor if need be. Because it's an intermittent fault, I think you probably just have a poor connection.
Check all your fuses just in case.
