Change the fluid and filter to ensure oil flow inside the transmission, if the problem persists, check and adjust the kickdown cable, hope this helps, let me know.

New starter problems on a 99 suburban 1500 4wd 350 vortec

pull the starter and check the ring gear teeth for burrs and mashed teeth. Ensure that the new starter has the correct number of teeth on the pinion as required by the ring gear or you will have the wrong gear ratio for your vehicle. There are many engines that use the same starter motor but are differentiated by the number of teeth on the pinion. I suspect that will be why there is hard noise and kick back as that is the out of mesh gears trying to work

O/D Off light goes off and on

the overdrive is the 4th gear. considering that it may not be an issue with your computer switching gears to fourth but an issue with the transmission itself. this light could be a signal instead of and engine warning light where the engine is fine but the transmission isnt.

automatics do not change gears
the gears are constant mesh and the ratio changes ( gears) is achieved by bands and clutches
change the transmission oil , and have the bands and clutches adjusted

It sounds like it is time to service your transmission. Even though your level and fluid looks ok your filter can be dirty (clogged). If you take it to be serviced have them drop your tansmission pan and replace your filter, do not let them talk you into using a procedure that sucks old fluid out and put new in. This will cost you more in the long run.
