Timing chain guards have broke off and fallen into the oil pan. the oil pick up is probably clogged. If so take the valve cover off to see if the oil is getting distributed.

The soulution is probably going to be expensive. It sounds like your oil pump is going bad or your piston rings are not sealing properly anymore. As far as the timing chain is concerned if the timing is still good and/or you are getting no backfires under acceleration then it is just showing normal wear. You probably need to rebuild the motor. I have a 91 22re 4wd 5spd when it first starts it makes a loud clacking noise that goes away after a few seconds what is that noise my rocker arm shafts and rocker arms are in good condidion

Sounds like it could be your timing chain. There is an automatic chain tensioner and over time when the chain gets a lot of miles on it the plastic guides and chain have to be replaced. Typically around 125,000 miles or more. You can remove the valve cover and look down the front of the engine to verify this. Look to see if the plastic guides are still in place and that the chain is not rubbing a hole in the front timing chain cover. Let us know your findings.

The 22re engine was infamous for having timing chain problems where the timing chain guide and tensioner would rub on the back of the waterpump. Check your timing chain to make sure all components are operating correctly and check the back side of the water pump. If it succeeds in tearing up that water pump cover you will coolant rush into engine and water lock the motor. More damage than checking and or replacing the timing chain and components.

Just replaced timing chain, guids, gears and tesioner in a 1991 nissan pickup with a z24 engine and still has a rattle when it worms up, not there when its cold would it be low oil pressure

Sounds like oil pressure.Try thicker oil and if the problem presists than
Check tension on chain and rotate by hand to see if everything lines up and there is no slack /
when installing oil pump ,ensure the oil ports line up/
check oil pressure with gauge

1990 toyota pickup with 22re engine and 235000 miles failed smog in cali.,not because of emissions but because timing was at 30 degrees instead of 10. It starts and runs well. I have not adjusted timing ....

Reset the engine timing by first jumping pins E1 and Te1 in the grey box under the hood. If the truck runs poorly set to proper timing, your timing chain has jumped a tooth. Does it rattle bad at idle? Id so, the guides are worn out, and the chain is stretched.

Overheating and water mixed in with oil

Blown head gasket will cause this so will a warped head, unproperly torqued bolts on the head, or even a crack in the block.
