It's very possible. Crud builds up over time due to the heat in the transmission. A fluid flush and filter change is a good idea. You may end up doing it twice - automatic transmission fluid is a great detergent and new fluid may dislodge crud inside the transmission, which will fill the new filter and clog it. Swap it out a second time and you're golden.

My 2003 Chevy 1500 will not shift past 2nd gear; can't drive on the highway. top speed 35-40 mph. The truck has 96,000 miles on it.

Any service engine light? May be a bad shift solenoid which are located inside the transmission pan.
Check for code(s) and come back with them.

When taking off the trans shifts slightly hard but not noticeable to other people. Could it be the trans filter or fuel filter, or just me?

Not the Trans Filter
Not the Fuel Filter
Not just you

TPS Sensor --worth testing

Any number of shift solenoids or solenoid pressure
control pack -- could be an issue

Could & should be changing trans & power steering
fluids every 3 years & not over 30,000 miles, on those
fluids. That won't fix anything, but may help them last longer

Make sure you use Mercon V Fluid

If the Spark Plugs have over 60,000 on them, install new ones

If the front O2 Sensors are OEM & 100,000 miles -get rid of them

2004 Montana shifts hard after being driven for about an hour. It has only occurred once, however, other times while sitting at a traffic light, when I press the fuel petal to go, it slips as if...

If the transmission has otherwise being acting OK, then I'd think that fluid and filter might take care of it. A shift solenoid might not be necessary, but it can't hurt. The only thing I differ with you on is your choice of Rislone. They make a great product, no doubt, but I've seen worse problems than you've described made to working smoothly using Lucas Oil.

My wife's Montana (01) has roughly the same miles, and it one time hit hard on a 2 / 3 shift while I was driving it.

I added Lucas (didn't change fluid, filter or anything) and it has worked just fine ever since.

I have a 1994 Chevy 2500 2 wheel drive. It shifts hard from 1st to second. If also will not drop down into a passing gear when you mash on the accelerator. Any Ideas?

do you have any trans related trouble codes, this trans has electronic shift control so u may have a solenoid sticking or a valve body issue, has the fluid ever been changed along with the filter? this is due every 60,000 miles, this has cured the problem in many cases like yours, fluid wears out (friction modifiers) and can cause harsh shifts.

Obd 2 codes p0761 p0420

P0420 - Catalyst System Low Efficiency, possible worn out cat converter, if truck is low miles may be sensor issue.

P0761 - Shift Solenoid C Performance or Stuck Off , at some time the transmission had a sticky shift (solenoid #C) solenoid, has the trans ever been serviced, if over 60,000 change the fluid and filter, this issue can be caused by a piece of dirt or clutch material from normal wear.
