did you ever replace the coolant when you changed it out. or flush the system of any small particles that could clogg it up?

then u are damaging it on installation, there is no other explanation for this

Is it wearing a hole through the heater core? Is it possible that something is just not line up, or is it a seam failure?

you dont tell me alot, but i would suspect an blown head gasket get a pressure test.adrian;;;
