I have encountered this problem before. Your transmission is probably the aisin warner 5550. These 5 speed automatic transmissions were used in some maximas from 2004 - 2006. The short answer is you will at least need a new & updated valve body. Because the valve bodies tend to have that problem, other internal transmission damage may result. I rebuilt and updated a valve body for a customer and the max ran fine after that. Even now months later, my customer's transmission is still running great and it has never jerked again. I already wrote a long post with pictures on enginegeeks.com. If you want to see it go here : http://www.enginegeeks.com/viewtopic.php?f=2&t=3
It's not the motor mounts and a transmission flush won't do the trick. These transmissions **** HARD!
probably not the trans. if the idle is high, and motor mounts are bad it will **** pretty hard when putting it into gear. For high idle look for a vacuum leak or the idle adjuster screw is not set properly. One way to check motor mounts, is to put it into park an watch the engine while you rock the car forward and backward real hard. if they are bad you will see the engine lift up a couple inches
