Sounds like a leaking inlet valve broken valve spring/ faulty valve/lifter prob. or a worn out camshaft if the exhaust cam has worn out the compessed gas cant escape so when the inlet valve opens the internal cylinder pressure blows back in to the inlet maifold if it was me i would do a cylinder compression test first that should help you to isolate the cylinder thats at fault
SOURCE: Where is the Fuel Filter located on 1988 Dodge Ram 50
Under hood, center, lower engine area, between fuel pump and carburetor, mounted in fuel line
SOURCE: 1998 Dodge Ram will not start -
check the asd relay in fuse box under hood look very close at contacts if replacing fuse does not fix wires in box may be burnt under neath
SOURCE: 98 dodge ram 1500 5.9 multiple cyl misfire. cyl 1
did you check to verify that you have the wires in the correct order possible distributor pick up bad
SOURCE: My honda bogs down, then pick back up, consistently
Hopefully the fuel filter. If you floor it and it has a delay or it won't reach high RPMs it's most likely the cause.

Ya. Timing belt might be the problem , intake valves staying open during compression stroke , but I don't think it would even run. Maybe just 1 intake valve. Have a look at the springs in the valve train.
