This code may be due to a broken intermediate band. TSB 04-24-17 was issued in December 2004 addressing this code. You can read the TSB here:

P0745 TRANSMISSION 2-3 Pressure Control Solenoid Malfunction
PULL THE PAN OFF THE TRANSMISSION AND REPLACE THE 2-3 SHIFT SOLENOID, IT IS BOLTED TO THE BOTTOM OF THE TRANSMISSION VALVE BODY
