Sort of sounds like the transmission has failed,
if it comes out of gear or you have to shut it off,
to get it into gear & shifting again for a few shifts

Transmission or altenator?

I would say change the alternator out. Start with the simplest and cheapest factor first. Without the alternator keeping the engine running its hard to put it any gear.

Car has a rattling noise and it is sluggish, will not go into next gear while accelerating

sounds like your timing is out if timing belt has not been serviced in last100000 km then it it possible that you have jumped a tooth on timing belt which means you should stop driving it before it jumps again and does damage to engine

if it is jumping gears, the gear box will not do that, it is more than likely your sedondary inside the transmission.
