2007 Honda Fit Base with 50K miles. Began hesitating a couple thou ago. It doesn't misfire like another comment. It can be reproduced by putting it in neutral and reving above 5K RPMs. The RPMs fluctuate/waiver in a consistent pattern (almost like a sign wave). Seems to get better with higher oct fuel, but not completely. Is it a fuel filter, computer mixture issue, or just plain old gunk?
No real answer to the first half of your question, but I do know that when you are revving the engine to 5,000 rpm in neutral you are hitting the engine's rev limit. It is most likely not related to your original complaint.

The CD player is mounted on very soft mounts to isolate road shock. These mounts fatigue allowing module to move lower from gravity losing alignment. Happens after about 50k to 100k miles. Honda owner Richard
If it's running rough, chances are that it's misfiring. If the Check Engine light is flashing/blinking, it is definitely misfiring. If that's the case, drive it as little as possible... Or straight to the mechanic because the more you drive it while it's misfiring, the higher the chance is of you ruining your catalytic converter.
