Sounds like it could be a mis-fire. At an idle, remove a plug wire to see if it stalls. If it does, re-install it and go to the next one. If you remove one and get no change, that cylinder is the problem. Test for spark.
this sounds like a vaccum line and can very hard to find but if you spray carb cleaner over them while its running and your motor starts to miss then you found it . hope that helps
If this engine is a VORTEC then you may have a problem with the internal fuel system. Under the top plenum there is a fuel reg and poppet valves that inject fuel into the cylinders. Sometimes the tubes that run fuel from the reg to the valves can leak and cause your problem. This cannot be found with pressure check because this occurrs after the reg. PS fuel mileage will go down. Let me know if this helps.Moe

1) Is the service engine soon light ever on when engine is running?
2) Per your description I would look at the ECT sending unit. It sends coolant temperature information the the Engine Control Module for proper fuel delivery for cold engine start. It may have lost its calibration.
