Do an RPM drop test to see if you have a lazy/dead cylinder. Here's how:

My little Ford Ka (Fiesta) was missing and vibrating badly at idle up until 2000 rpm. The spark plug cables were the problem. Also it's a good time to change the oil and the spark plugs too.

I'm sorry I didn't read that closely. The next step would be to check the timing and adjust if necessary. After that you need to check the fuel pressure but that might call for a real mechanic. At least at that point you've eliminated the basic possible issues.

    Good Luck!

Car reaches 60-65 MPH, then slows down to 35 or 40m/h....changed mission filter and oil, and speed sensor....tuned up

on the off chance check the condition of the cat converter as it may be blocking the exhaust and acting as an exhaust brake above certain speeds
run the fault codes as well to check for other sensor problems

Van running like its missing

the problem is not in the plugs , wires , cap , rotor but in dirty injectors or injector controls and wires
use a quality injector cleaner for a few tanks of fuel
ECM controlled cars do not a tune up as the computer does that 60 times a minute so all you need to do is to run the fault codes and fix the faulty sensors
fault codes are being recorded all the time and only the serious engine or transmission faults put the engine light on

I have a 1987 Maserati BiTurbo Si (injected). Engine ran very well but now the idle is falling off. There is a 'miss' and the low idle is lopey (bumpity-bumpity-bumpity) until it falls off. With the rpm...

It might be an engine management system (the MABC- Maser Auto Boost Control) gone wonky. That or a faulty O2 sensor or even a poor fuel filter (or 2, since most US SIs have a redundant one). Even a bad cat could do it. You could spend some time on this...

Hesitation, stalling like out of gas

Clogged Catalytic Converter. What happens is as you push down harder on the accelerator you force more gas into the system causing a slightly richer mixture and more exhaust flow is neccessary. The easiest way to look for a clogged cat is when it's dark out take it for a ride causing it perform it's problem look under the vehicle if the cat is literally glowing red hot you either have a clogged cat or an extremely rich fuel mixture.

Yes, it is possible that the exhaust is plugged, and more specifically, it could be at the cat. converter. You would need someone to disconnect the exhaust system where the header pipe connects to the exhaust manifold, then drive it. If it preforms good, check converter or muffler. I have seen both at fault.
