Look at the driver side visor the shift pattern is printed there. it should be also on the shift lever itself . I think 2 wheel high is all the way forward then back 1 is neutral back 1 more is 4w low and all the way back is 4w high

Transmission?

This issue sounds like dried seals. it is unlikely a filter issue, but no reason to exclude that. Ford uses coarser screen, and does not plug as often.
Consider "mechanic in a can" transmission conditioner, aiming to revive seals.
In any event, the unit may be happier after running some miles.
If that does not work, it is trans surgery time.

Goes to 4000 rpms to 5500 rpms before changing from 1st to 2nd g

Ford : back the truck up 10ft put truck in neutral and disengage four wheel low. (You can't drive in four wheel low cause its a pulling gear and you won't do over 3-5 mph.)
Nissan: start truck see if it idles normal. If so rev motor and check throttle for sticking and check EGR valve make sure its not stuck open or malfunctioning when you drive it..
